Self Management Programmes (SMP) are available for adults living with a long-term health condition, or for those in a caring role. SMP aim to improve your quality of life by giving you confidence, skills and information to help manage your long-term condition or caring role better on a daily basis.

All courses are FREE and run for 6 weeks.

Currently our community programmes have been suspended due to COVID 19, however we now offer virtual and telephone courses.

Normally programmes would run all year round at different venues throughout the Dudley borough. Each course is tutored by trained volunteers who themselves are living with a long-term health condition. Morning, afternoon and evening courses are available.

Topics include:

  • Fatigue and getting a good nights sleep
  • Coping with difficult emotions/depression
  • Physical activity, preventing falls and improving balance
  • Relaxation and breathing techniques
  • Healthy eating
  • Communicating with friends and family and health professionals
  • Pain management and extreme tiredness
  • Problem solving
  • Goal setting

By taking part in a programme you will:

  • Learn new skills to help manage my health condition and improve my quality of life
  • Develop confidence in the daily management of your specific condition
  • Meet others and share similar experiences
  • Learn how to develop more effective relationships with healthcare professionals
  • Be realistic about the impact of my condition on myself and my family
  • Plan for the future

For people living with a long-term health condition we offer the ‘Expert Patients Programme’ (EPP). For people who have a caring role we offer ‘Caring for Me and You’.
