Self Management Programmes (SMP) are available for adults living with a long-term health condition, or for those in a caring role. SMP aims to improve your quality of life by giving you confidence, skills and information to help manage your long-term condition or caring role better on a daily basis.  This service is delivered by Dudley Empowerment Partnership.

SMP is a FREE 6 week course with different types of courses available

Booking is essential

Delivery methods include:

  • Face to face in COVID secure community venues
  • Virtually
  • Over the telephone

(Virtual and telephone courses have an additional introductory week)

Each course is tutored by trained volunteers who themselves are living with a long-term health condition.

All participants are provided with course resources including a self management handbook.

Topics include:

  • Fatigue and getting a good nights sleep
  • Coping with difficult emotions/depression
  • Physical activity, preventing falls and improving balance
  • Relaxation and breathing techniques
  • Healthy eating
  • Communicating with friends and family and health professionals
  • Pain management and extreme tiredness
  • Problem solving
  • Goal setting

By taking part in a programme you will:

  • Learn new skills to help manage my health condition and improve my quality of life
  • Develop confidence in the daily management of your specific condition
  • Meet others and share similar experiences
  • Learn how to develop more effective relationships with healthcare professionals
  • Be realistic about the impact of my condition on myself and my family
  • Plan for the future
