Healthy Hubs Parks Exercise Scheme

What is it?

Clients are given the choice of participating in group activities or exercising independently. The type of activities include:

• Walking groups

• Cycling groups (using your own bike)

• Outdoor exercise sessions

  • Patient to take the form to the Healthy Hub Park of their choice

Additional considerations

Non-health care professionals – Blood pressure between 160/90 and 180/110 The client should be sent to Action Heart for observation, retest, possible exercise tolerance test. Telephone Action Heart 01384 456111 ext.1470 or use referral form A and follow referral process from higher and moderate people to Action Heart.

Clients with blood pressure above 180/110. Refer client back to their GP for guidance on the same day.

Health care professionals – can recommend physical activity for their patient. Ensure the health care professional referral form is used.
