ShapeUp4Life – Open

ShapeUp4Life is a lifestyle change that focusses on habits and behaviours, combining physical activity, balanced diet for a long-term behaviour change. It will support patients to lose weight, and maintain a progressive weight loss over a 12 week programme.

Who is it for?

People 18 years and older who have a high body mass index (30 or over, or 27.5 and over for Asian, Black African/Caribbean heritages). The patient must live or work in the Dudley Borough, or registered with a Dudley Borough doctor.

Remember they need to be motivated to make healthy lifestyle changes.

  • Huntingtree Park Activity Centre (Halesowen, B63 4HY) Saturday’s 9 – 10:30am or 10:30am – 12pm
  • Mary Stevens Park Activity Centre (Stourbridge, DY8 2AB) Wednesday’s 1.30-3pm or Thursday’s 6 – 7:30pm
  • Netherton Park Hub (DY2 9EX) Friday’s 1-3pm
  • Plaza Mall, Halesowen High Street (B63 3BA) – Tuesday’s 10:00 – 11:30am or 12:30 – 2pm
  • Sedgley Hub Age UK (DY3 3SG) Thursday’s 10:00 – 11.30am
  • Silver Jubilee Park Activity Centre (Coseley, WV14 9SZ) Wednesday’s 1:30 – 3pm
  • Wordsley Community Centre (DY8 5BN) Friday’s 10-11.30am
  • Zoar Church, Gornal (DY3 2PG) Thursday’s 10 – 11:30am

Additional considerations

This programme is not suitable for anyone who is/has:

  • Pregnant or Breastfeeding
  •  Eating Disorder
  • Underlying medical cause for obesity and would benefit from more intensive clinical management
  • Significant co-morbidity or complex needs as identified by your GP or other healthcare professional

Blood pressure between 160/90 and 180/110 The client should be sent to Action Heart for observation, retest, possible exercise tolerance test. Telephone Action Heart 01384 456111 ext.1470 or use referral form A and follow referral process from higher and moderate people to Action Heart.

Clients with blood pressure above 180/110. Refer client back to their GP for guidance on the same day.
