Action Heart can now maintain a supportive network for graduates from their Self Management Programme, particularly for those for whom the Programme has potentially saved them from loneliness and isolation. The Graduate Network provides longer term support for graduates and signposts individuals to other services as appropriate to their needs. SMP Graduate Network encourages graduates to be more self reliant, resilient and increases social connectedness within their local community.
